Academic studies from tertiary institutions paint an equally stark picture, with one survey at a government-funded North Indian hospital finding that more than 90 percent of resident doctors and faculty reported some level of burnout on validated screening tools.

Doctor Burnout in India: Recognising It and What Institutions Are (and Aren't) Doing

What the Data Actually Shows

What Drives It, Beyond Hours Alone

Recognising Burnout as Distinct From Ordinary Fatigue

What Institutions Are, and Are Not, Doing

Conclusion

Researched Resources

What the Data Actually Shows

In the Post Graduate Institute of Medical Education and Research study, 30.1 percent of surveyed doctors screened positive for depression on the PHQ-9 scale, and 16.7 percent reported suicidal ideation, figures drawn from a supposedly well-resourced tertiary academic institution rather than an under-staffed rural facility. Resident doctors reported meaningfully higher rates of stress, depression, and burnout than faculty, consistent with the disproportionate service and duty-hour burden residents typically carry.

A broader post-pandemic narrative review found that burnout rates among Indian healthcare workers have continued rising rather than normalising after the acute pandemic period ended, alongside elevated rates of anxiety and depression that are worsening healthcare delivery quality as a downstream effect, not just an individual wellbeing concern.

What Drives It, Beyond Hours Alone

Long duty hours are a consistent factor, but they are not the whole story. Studies point to a wider set of contributors including experiencing verbal or physical abuse from patients or caregivers, a perceived lack of empathy from senior colleagues, limited engagement in recreational or restorative activities outside work, and, for interns specifically, the emotional weight of managing patient loss and infection risk during high-acuity postings such as emergency and trauma care.

The 2025 Doctors’ Day survey adds a further dimension often missing from clinical burnout literature: nearly half of respondents cited worry about the broader failure of public healthcare infrastructure as a source of psychological strain, suggesting that burnout in the Indian context is tied not only to individual workload but to systemic anxiety about the sector doctors work within.

Recognising Burnout as Distinct From Ordinary Fatigue

Burnout is typically assessed across three dimensions: emotional exhaustion, depersonalisation or cynicism toward patients and work, and a diminished sense of personal accomplishment. This distinguishes it from simple tiredness, since a doctor can be physically rested after a day off yet still show depersonalisation, characterised by a flattened, detached attitude toward patients that was not present earlier in their career, or a persistent sense that their work no longer feels meaningful regardless of clinical outcomes.

Because these symptoms often develop gradually and doctors are professionally socialised to push through fatigue, burnout frequently goes unrecognised by the affected doctor themselves until it manifests as a more acute problem, such as a medical error, an outburst with a patient or colleague, or a mental health crisis that finally prompts attention.

What Institutions Are, and Are Not, Doing

Where institutional response exists, it tends to be recommendation-heavy and implementation-light: academic studies consistently call for regular institutional-level mental health screening, structured stress management workshops, and revisiting duty-hour policies for residents, but few Indian hospitals or medical colleges have moved these recommendations into standing, funded programmes rather than one-off wellness initiatives.

A meaningful gap remains between what research repeatedly recommends, confidential and accessible mental health support integrated into training and employment, protected rest time, and structured supervision that includes attention to trainee wellbeing, and what most institutions currently provide, which in many settings is limited to informal peer support or ad hoc counselling referrals without dedicated funding or protected time built into rosters.

Conclusion

The data on doctor burnout in India is no longer ambiguous or anecdotal; it is well documented across multiple independent surveys and academic studies, and it is getting worse rather than better in the post-pandemic period. The gap that remains is between documentation and action: institutions that treat burnout screening and structured support as a standing operational responsibility, rather than an occasional wellness initiative, are still the exception rather than the norm.

Researched Resources

1. Long hours, low safety: Over 80% of India’s doctors face burnout

2. Psychological problems and burnout among medical professionals of a tertiary care hospital of North India

3. Exploring post-pandemic burnout and mental health among healthcare workers in India

4. Under the stethoscope: Unveiling the surge for suicides and mental health concerns among medical professionals in India

Disclaimer: This article is for general informational and educational purposes and discusses doctor burnout in India as a workplace and systemic issue based on published survey and research data available at the time of writing; individual experiences vary.
